Specialist absence notification

The Specialist Absence Notification Form is easily available to doctors and practice managers to allow communication between medical practices and the Mater Private Hospital Townsville.  This information will assist in facilitating smooth changes and updates to theatre lists and patient flow into and out of the Hospital.

Holidays, professional and personal leave is a part of every working relationship and here at the Mater Private Hospital Townsville, we encourage you notify us as soon as possible. 

We understand that on occasions emergencies arise and sometimes short notice may be beyond your control.  We encourage you to provide at least four weeks' lead time.  

Once this form is submitted, your theatre bookings (if applicable) will be automatically cancelled.
